Are you ready to awaken your inner potential and live life to the fullest?

Unlike fad diets that result in a frustrating cycle of weight loss and regain, the Lifestyle Change Program incorporates evidence-based practices from those who have succeeded with long-term weight loss. Our four month program will guide you step by step, helping you shed pounds while reversing chronic diseases like type 2 diabetes, high cholesterol, and high blood pressure.

Program overview

With personalized medical oversight from a multidisciplinary team, combined with group classes, the Lifestyle Change Program is unique among weight loss programs. Clients are supported by a team of clinicians including a physician, a dietitian, and a behavioral therapist, each with distinct skills and training to maximize every participant’s success.

The Lifestyle Change Program strives to help participants make sustainable lifestyle changes by approaching wellness in a whole-person manner, as opposed to a quick-fix diet. By combining instruction on nutrition, physical activity, stress management, mindfulness, and the confidence to make healthy lifestyle changes, our participants are set up for long-term success.

2024 Start Dates

 

The Lifestyle Change Program will start new 15-week programs during the following timeframes:

  • January 2024
  • May 2024
  • September 2024

Program Details

$700 (payment plans available)

  • 16 group classes.
  • A binder with new handouts at every class to guide you through the 16-week program.
  • Body composition testing at baseline and the conclusion of the program, providing a breakdown of muscle mass, fat percentage, basal metabolic rate, hydration status, and more.
  • Labs to include a full cholesterol panel and hemoglobin A1C (diabetes indicator) at baseline and the conclusion of the program
  • Four visits with our providers (physician, registered dietitian, and behavioral therapist).
  • Follow-up visits with providers, if additional support is desired.
  • Discount on Wellness Center classes (cooking, mindfulness, and fitness).

$250 (payment plans available)

  • 16 group classes.
  • A binder with new handouts at every class to guide you through the 16-week program.
  • Body composition testing at baseline and the conclusion of the program.
  • Discount on Wellness Center classes (cooking, mindfulness, and fitness).

 

Optional add-ons:

  • Visits with the dietitian or behavioral therapist ($50 for a one hour visit, or $75 for a 2-visit package)
  • Visits with the physician (billed to insurance)
  • Lab draws (billed to insurance)

We are covered by the following organizations:

  • Sky Lakes Medical Center. If you are an employee of Sky Lakes Medical Center and are covered by Sky Lakes insurance, the Lifestyle Change Program will be fully covered by Sky Lakes.
  • Sky Lakes Insurance. If you are the spouse or child (ages 16+) of a Sky Lakes employee and are covered by Sky Lakes insurance, the Lifestyle Change Program will be fully covered by Sky Lakes.
  • Oregon Health Plan. If you are covered by the Oregon Health Plan, the Lifestyle Change Program will be fully covered by Cascade Health Alliance, pending approval.

 

We welcome you to submit claims to your insurance company for reimbursement. Please contact the Wellness Center for details.

There are two main criteria one must meet:

  • Desire. You MUST have the desire to change. Participants must be thirsty for better health. Daily habits can be deeply engrained and adopting new, healthy behaviors is challenging. The journey to wellness takes time and determination.
  • BMI. Participants must have a body mass index (BMI) greater than 30 or a BMI greater than 25 with at least one weight-related disease, such as prediabetes, type 2 diabetes, abnormal cholesterol, high blood pressure, or obstructive sleep apnea.
