MyChart is a free and secure online tool that connects you to portions of your medical record.

 

Patients no longer need an activation code to sign up for MyChart.

 

If you have questions about how you can be using MyChart, ask your care team or call 541.274.6300.

 

Patients at any of the Sky Lakes clinics and Cascades East Family Medicine Center can access MyChart using information from their providers. Contact your provider for more information.
