Community Grants

As our nation grapples with the spread of COVID-19, SLMC is being vigilant and redirecting all of our efforts to help protect our community.

As a result of these efforts we are pausing all community grant fund distribution at this time as we reassess our giving potential. We will post all updates as they become available. Thank you for your patience.

Sky Lakes Medical Center is proud to be a community hospital that gives back. We are very supportive of collaborative approaches to addressing community health issues and we welcome proposals developed by partnerships and coalitions with clearly articulated strategies. Each applicant may apply for funds up to $5,000 each quarter.

Priority Areas

Funding decisions are based on how well proposals align with one of Sky Lakes Medical Center's priorities. Your proposal will need to target and have a meaningful impact on at least one of these priority areas. Multiple connections between priority areas are encouraged.

  1. Health, wellness, and safety activities and programs
  2. STEM&M (Science, Technology, Engineering, Math and Medicine)-related education events and programs
  3. Specific economic development programs that are consistent with the Sky Lakes mission
  4. CHIP: Length of Life and Quality of Care (Behavioral Health)
  5. CHIP: Quality of Life and Health Behaviors (Physical Health)
  6. CHIP: Access to Care (Oral Health)
  7. CHIP: Maternal and Child Health
  8. CHIP: Social Economic Factors
  9. CHIP: Physical Environment

CHIP refers to the 2019-2021 Community Health Improvement Plan, a long-term, systematic effort to address health issues and concerns, and the factors that influence them. The CHIP is used by health care agencies, in collaboration with community partners, to establish priorities and to coordinate activities and resources to improve the health and well-being of community members, and the overall health status of the community. Eligibility

Our Community Grants process is for local non-profit (501 (c)(3) organizations) or civic/community organizations from the Sky Lakes Medical Center patient service area, including Klamath Falls and surrounding areas.

Grant Process

Applications are collected quarterly (see deadline below) and reviewed by the Sky Lakes Charitable Giving Committee. Applications are reviewed based on how they best meet the priority areas listed above. Applicants will be notified by the end of the following month regarding their application status. Example: September 30th deadline applicants will be notified by October 30th. (SLMC fiscal year runs October 1st - September 30th)
