Patients and Visitors - For Patients

When You Arrive 

Parking for Day Surgery patients is just off Bryant Williams Drive between the hospital and the Medical Office Building.

 

When you arrive at Sky Lakes Medical Center, check in at the Information Desk just inside the main entrance. One of our friendly Guild volunteers will direct you to an Admissions clerk, who will verify your personal information. You’re welcome to bring a friend or family member with you during the admission process.

 

To help the admission process proceed as quick and easy as possible, please bring: 

 

  • Your health insurance plan card
  • Your prescription insurance plan card
  • Personal identification (such as a driver's license)
  • Medical records, X-ray films, or prior test results (if your physician has requested them)
  • Written letter of authorization from your insurance carrier (if you are covered by worker's compensation)
  • A copy of your advance directive or living will
  • A prepaid phone card if you plan to make toll or long distance calls. 
  • Prepaid phone cards are available at The Link Gift Shop on the first floor.
  • Please bring only essential personal items, such as glasses, dentures, hearing aids, cane or walker.

 

Once admitted, you will be given an identification bracelet with your name and a Sky Lakes Medical Center medical record number. For your safety, do not remove the bracelet until you are discharged from the hospital.

To prevent health care errors, patients and families are urged to Speak Up, the Joint Commission-sponsored program that provides simple advice on how you can make your care a positive experience. 

 

Things to keep in mind during your stay:

 

  • Your illness may make you weaker than you might expect, and the effects of surgery or medications may cause you to be confused or disoriented.
  • Always follow instructions from your doctors, nurses and therapists.
  • Check with your nurse before changing the position of your bed because your doctor may have specific reasons for your bed position.
  • The side rails on your bed may be up after surgery or when certain medications are used. Call your nurse to have them lowered.
  • Ask for help when getting in and out of bed. Get out of bed slowly and rest on the side of the bed before you stand up.
  • Wear slippers when you are out of bed. Walking in socks or bare feet can be unsafe.
