During Labor   
    You may request a confidential admission, 
       if desired
    You may decide who visits

After Delivery
    Baby’s identification bands and crib card will have 
        your name on them
    You choose who visits
    You make all decisions about medical care for the 
        baby
    The decision on where 
         your baby stays is yours.
    You may select to have your baby stay: 
         in your room
         in the nursery
         in another room with adoptive parents
         
Birth Mementos
    Baby pictures
    Footprint card
    Tee shirt
    Crib card and identification band

Going Home
    When?  This is up to you and your doctor.
    What about baby?  You have three choices:
        Baby can leave the building with you
        Baby can leave the building with an attorney
        Baby can leave the building with a designated person from an adoption agency

    Release of information form
    If baby is not leaving the building with you , consent form for baby to leave with someone else
