Who qualifies?
Sky Lakes Medical Center will provide at no cost non-elective services to patients whose household income falls at or below 150 percent of the Federal Poverty Level. Patients whose income is between 150 percent and 225 percent of the Federal Poverty Level and who are unable to pay their bills are eligible for a partial write-off of their accounts. 

If your income falls below 100 percent of the federal poverty level, you may be eligible for assistance through the Oregon Health Plan. For information about the Oregon Health Plan, contact Adult and Family Services 700 Klamath Ave., Klamath Falls, 541-274-5511.

What services are discounted?
Only non-elective services are eligible for discounts under this financial assistance policy. Elective procedures are not covered under this policy. Eligible services are non-elective hospital and Cascades East Family Practice Clinic services. Elective procedures, walk-in pharmacy, physician services, and services required as a result of the patient’s criminal behavior are excluded.

Covered and non-covered services, as defined by a patient’s insurance policy, will be considered eligible. Gastric bypass surgery and other cosmetic procedures are elective procedures.

What discount levels are offered?
See the Federal Poverty Level table for the income levels and corresponding discounts available. The discounts apply only to the portion of the bill for which the patient is responsible.

Who benefits?
Assistance under this policy is intended solely for the benefit of the patient and the patient’s family and does not relieve third parties of liability for payment.
