If you have concerns, problems, or complaints about patient safety, the quality of care or service that you or a loved one received in our care, you are encouraged to speak directly to the providers, or an appropriate manager or supervisor.
If your concerns are not resolved to your satisfaction, or if you would like the help of someone not immediately involved, our Patient Advocacy Staff are available.
Concerns regarding patient care and safety may be communicated to Sky Lakes Medical Center Patient Advocacy staff by calling 541-274-4516 during regular business hours.
