An Advance Health Care Directive is a document that describes the extent of medical treatment you do or do not want to receive should you become unconscious or too ill to communicate with your healthcare workers.
Though the topic may be difficult to discuss, it is important for your wishes to be honored. We recommend that you discuss the advance health care directive with your spouse, family members, spiritual advisers, and/or healthcare provider, while you are feeling well and thinking clearly, preferably prior to admission, although anytime is fine.
The directive allows you to appoint an individual to make healthcare decisions for you if you are unable to do so yourself. This practice is completely voluntary.
