Patient Satisfaction 

It is our collective goal at Sky Lakes Medical Center to provide the best care possible to every person who comes through our doors.

 

In order to maintain our high standards of excellence, we regularly ask our patients and their families to share information about their experiences while here. We do this by mailing surveys as well as by asking questions in person or by telephone.

 

If you receive a survey, please return the completed form in the postage-paid envelope.

 

Request a survey in the mail if you did not receive one following your last stay. Remember to tell us the department you visited. You can also telephone 541-882-6311 to request a survey or request a survey via email to Webmaster.  

 

If you have a concern or complaint about the care we deliver, we’d like to hear from you. Call our Quality Management Department at 541-274-4772. You can also contact the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). 

Visitors and patients' family members also can volunteer their observations.
