Home Health

Home Care Services
Home Health, a division of Sky Lakes Medical Center, has provided quality home health care since March of 1984 and is located at 2605 Crosby Avenue. Our services are available to clients within 33 miles of Klamath Falls, in both Oregon and California.

 

Skilled services: Quality Care in Your Own Home
At Home Health, a team of skilled nurses, therapists, and aides help clients remain in their own homes, shorten hospital stays and reach individual goals of independence and health.

 

Patients recover faster and are happier because their care is centered on familiar routines and adapted to their environment. Care can be personalized and directed at meeting individual needs, while patients enjoy having family and pets nearby and experience the comfort of sleeping in their own beds.

 

Home Health provides the assistance needed by patients recently discharged from the hospital, who are not ready to assume full responsibility for their own care.

 

Many patients can benefit from Home Health services whether they are on the mend or facing chronic illness. Insurance companies acknowledge the financial benefits of home care and cover many of the services offered. Home Care is an excellent option for patients and families needing assistance with health care needs.

 

Range of Services
Whether you have a new condition or some changes in a chronic condition you may qualify for Home Health services. Ask your doctor about whether Home Care would meet your health care needs.

 

The following services are available to patients living within a 33-mile radius of the Home Health office:

 

  • Nursing for skilled procedures, teaching, and skilled assessments
  • Rehabilitation services — Physical Therapy, Occupational Therapy,
           Speech and Language Pathology for skilled therapy procedures, teaching
           and skilled assessments 
  • Medical Social Work for counseling, community resource planning
           and assessment of social and emotional factors. 
  • Home Health Aide Services for assistance with personal care and may
           include light housekeeping

 

Special Services
    Wheelchair Clinic -- Brings together the patient with their vendor
       of choice and the therapist for evaluation and prescription to meet
       special seating needs.


    Wound, Ostomy and Continence Nurse -- a nurse specialist for assessment
       and treatment of patients with complex wounds, and ostomy and continence
       issues. Management of diabetic foot for problems and prevention. Coordination
       of care with physicians and hospital discharge planners. Provides more in depth
       focus on nutritional needs of patients with wounds, ostomies or continence issues.

Personal Care Services
Through genuine caring companionship families can have peace of mind knowing that well trained, CPR-certified Home Health staff are providing for the needs of their loved one. A Registered Nurse is provided at no extra cost to the patient/family to conduct the initial assessment to set up appropriate services, as well as being available to consult on a variety of health-related topics.

 

Patients, family members or physicians can request the services and some services may be covered by private insurance policies. Rates for the services are affordable and structured to make it easy for private pay if insurance is not available.

 

Services Include
   
Home Health Aides for assistance with bathing, dressing, walking and transferring,
       assistance with meal preparation and eating, incontinence care/toileting,
       shopping and light housekeeping.

 

    Sitters for assistance with grocery shopping, meal preparation respite for caregivers,
       safety and security, companionship and light housekeeping. Sitters may be arranged
       for short visits or for round-the-clock care.
