MEDICAL/DENTAL
INSURANCE

 

 

Hospital pays 80% for full time employee and 70% for family. Rates vary based on employee status.

 

 

1st of month following date of hire. Must be .5 or greater regular status.

 

 

LIFE INSURANCE

 

 

$50,000 term life policy, plus accidental death and dismemberment paid by hospital.

 

 

1st of month following date of hire for .5 or greater status.

 

 

ADDITIONAL LIFE
INSURANCE

 

 

$10,000 - $500,000 term life policy for employee and up to $250,000 for spouse.

 

 

1st of month following date of acceptance for .5 or greater status.

 

 

PENSION

 

 

Basic retirement income plan paid by hospital equal to 5% of monthly earnings.
100% vested after 5 years.

 

 

Plan anniversary date after 1 year service
(minimum 1000 hrs).

 

 

LONG TERM
DISABILITY

 

 

Disability income protection equal to 66 2/3% of basic monthly earnings with maximum benefits paid to age 70.
After 150 days disabled.

Tax shelters a voluntary portion of income from tax and adds to retirement income.
